Milestone Family Medicine is open Monday through Thursday, 8:00 am to 5:00 pm,
Friday 8:00 am to 4:00 pm. Please call 770-414-0337 for an appointment.
In order to continue provide you with prompt and efficient service, please notify our
office at least 24 hours in advance if you need to reschedule or cancel your appointment.
Payment Information
Milestone Family Medicine will file your medical claim for you if you have medical
coverage. We participate in most insurance plans.
Co-Payments and deductibles must be paid at the time of service. We accept cash,
checks, VISA and MasterCard.
There is a $25.00 fee for all returned checks.
